RELEASE WAIVER / RISK AGREEMENT
I understand clearly that by agreeing to this release I will be forever prevented from suing or otherwise claiming against Nritya Kala Mandir, its Director(s), Instructor(s), employee(s), agent(s), member(s) for any loss or damage connected with property loss or personal injury that me/ my ward may sustain while participating in the program, whether or not such loss or injury is caused solely or partly by the NEGLIGENCE of Nritya Kala Mandir, its Directors, Instructors, employees, agents, members. By agreeing to this release waiver, I also give Nritya Kala Mandir the authority to post pictures of me/my ward in the media sources available (e.g. print media, web media, visual media, television programs and any other such communication media). I understand that i/my ward will not be allowed to participate in the program unless this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT is signed. By agreeing to this document i approve that I am of sufficient age and mental capacity to sign this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT and understand all the points stated in the agreement and in my complete senses am agreeing to this document.
I am freely and voluntarily executing this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT.
